40M Teams Note: USL (United Soccer League) is using the same insurance for 2007, and our insurance agents, NASF and Francis Dean Associates have assured us (the USL and MCSSA) that if the USL sanctions MCSSA games, the same insurance will apply for both leagues. Thus, 40M teams that have 2005 insurance through the USL will already be covered for their MCSSA practices and games, and need not purchase the insurance again. 11/12/04 - Updated MCSSA Injury Report Form posted below to add email address fields and change medical claim submission deadline after injury from 90 days to one year. 5/24/04 - 2004 list of Additional-Named-Insureds (typically field owners) has been updated to reflect certificates ordered as of 4/21/07. Teams whose field owners require such a certificate MUST provide full organization name and mailing address info on their applications forms! Upon receipt, the MCSSA mails the certificate directly to the named organization. An MCSSA Injury Report form is posted below. If an excess medical claim form is needed, team manager must fill out and submit the Injury Report Form below (instructions are provided at bottom of the form). Upon receipt by the MCSSA, if requested (by check box on the form), the MCSSA will mail MCSSA-approved excess medical claim form to the injured player, who must complete and mail to insurance claim processor (info is on the claim form) within one year of the injury. Later, after all treatment bills and unpaid primary medical insurance (if any) statements are available, claimant must submit that info to the insurance claim processor.
